    探讨希望护理模式在老年冠心病患者护理中的应用效果。

    方法

    选取2016年8月至2017年8月海阳市人民医院收治的老年冠心病患者160例作为研究对象,将所有患者随机分为观察组与对照组,每组80例。对照组患者给予常规护理干预,观察组采用希望护理模式,比较两组患者心理状态、治疗依从性及生命质量改善情况。

    结果

    护理前两组SAS、SDS评分比较,差异无统计学意义(P>0.05),护理后观察组SAS、SDS评分均低于对照组,差异有统计学意义(P<0.05);观察组患者在配合治疗、按时吃药、配合调查、听从医生建议等方面依从性均高于对照组,差异有统计学意义(P<0.05);治疗后观察组患者生命质量评分均高于对照组,差异有统计学意义(P<0.05)。

    结论

    将希望护理模式应用于老年冠心病患者护理中能够有效降低其焦虑、抑郁程度,提高对治疗依从性,同时改善患者的生命质量,可为临床治疗奠定良好的基础。

    Cause analysis and nursing countermeasures complications of patients with atrial fibrillation radiofrency ablation

    Objective

    to investigate the reasons of causing complications in patients with atrial fibrillation radiofrency ablation and to explore the related nursing countermeasures.

    Methods

    Since august 2014 to september 2016 our hospital 86 cases patients with atrial fibrillation radiofrency ablation were analyzed retrospectively. analyzed the reasons of causing pressure sores, pain, uroschesis, vagus reflex, cardiac tamponade and so on.Active related nursing strategy was adopted in order to reduce complications.

    Results

    20 patients had complications. 2 cases had pressure sores, 12 cases had pain, 3 cases had uroschesis, 2 cases had vagus reflex, 1 case had cardiac tamponade. all cases recovered by active treatment.

    Conclusion

    The occurring of complications should be carefully observed, related treated timely and nursing countermeasures which can prevent and reduce the occurring of complications effectively, ensure the radiofrequency ablation effectively.

    Influencing factors of false positive in treadmill exercise test

    Objective

    To explore the influencing factors of false positive in treadmill exercise test.

    Methods

    222 treadmill exercise testing positive patients who were undergone coronary angiography within 1 month were studied. Patients were divided into two groups: coronary heart disease group (CHD group, n=143) and non coronary heart disease group (non CHD group, n=79). Clinical manifestations, combined diseases, coronary arteries situations, ultracardiography and treadmill exercise testing datas of these patients were collected and analyzed.

    Results

    Female gender (P=0.003, OR=11.112, 95%CI 2.258-54.784), age (P=0.016, OR=0.916, 95%CI 0.854-0.984), Bruce level (P=0.006, OR=3.453, 95%CI 1.424-8.373) and ejective fraction (P=0.029, OR=1.103, 95%CI 1.010-1.204) were independent influencing factors of treadmill exercise testing false positive.

    Conclusion

    Female gender, age, Bruce level and ejective fraction were independent influencing factors of treadmill exercise testing false positive. Meanwhile other atherosclerosis risk factors and parameters of exercise testing (besides Bruce level) could not provided reference to the judgement of false positive.

    Distribution of Plasma Brain Natriuretic Peptide in Elderly Patients with Chronic Heart Failure after Anti-heart Failure Treatment and Its Influencing Factors

    Objective

    To analyze the level of plasma brain natriuretic peptide (BNP) and the related factors in the patients with chronic heart failure after anti heart failure treatment.

    Methods

    The subjects selected from our hospital in 45 cases of elderly patients with chronic heart failure from October 2014 to December 2016 in Guizhou Cancer Hospital, the clinical data of all patients were analyzed retrospectively, and the patients as the observation group, and 45 healthy persons in our hospital were selected as control group in this study. Two groups of subjects of systolic blood pressure, diastolic blood pressure, pulmonary congestion, pulmonary hypertension, compare the left ventricular ejection fraction, and affecting factors of the related results were analyzed.Comparison of the patients in the observation group before after anti heart failure treatment and treatment level of BNP patients, and to assess the resistance the treatment of heart failure.

    Results

    There were significant differences in systolic blood pressure, diastolic blood pressure, pulmonary congestion, pulmonary hypertension and left ventricular ejection fraction between the two groups(P<0.05). The levels of BNP in the observation group were compared before and after treatment, and the difference was statistically significant before and after treatment (P<0.05).

    Conclusion

    Hypotension, pulmonary congestion and other related factors are independent risk factors of chronic heart failure. After the treatment of anti heart failure, the BNP level of patients can be significantly improved.

    Objective

    To investigate the analysis of patients with acute heart failure patients, with the effect of clinical nursing path in the process of treatment.

    Methods

    The number of patients in this study were 70 cases, hospitalization time for the July 2016-2017 year in February, all patients were divided into two groups for comparison, observation group and control group, respectively. The implementation of routine nursing and clinical nursing path, comparing the effect.

    Results

    In terms of nursing effect, the effective rate of the observation group was 8.57% higher than that of the control group. Meanwhile, the satisfaction degree of the observation group was 8.58% higher than that of the control group, and the life treatment of the observation group was higher than that of the control group, P<0.05.

    Conclusion

    In the acute heart failure patients, nursing intervention in the treatment of the path effect is ideal, can effectively alleviate the clinical symptoms of the patients, improve the patient's condition, but also can improve the quality of life, accelerate the recovery of patients, improve the overall quality of clinical service.

    Objective

    The aim of this study is to analyze the quality of life (QOL) among patients with Infection associated with cardiac implantable electronic device (CIED). And to explore the influence of psychological status on QOL.

    Methods

    From October 2015 to December 2016 in the heart center of Peking University People′s Hospital hospitalized patients with CIED infection in 120 cases, the application of Short Form Health Survey(SF-36), self rating Depression Scale (SDS), self rating Anxiety Scale (SAS) was used to investigate the psychological status of patients, quality of life, and in which 10 patients were interviewed.

    Results

    The QOL among patients with a CIED infection were significantly lower than that of healthy people. The status score of anxiety and depression, duration of infection and the education background are the influence factors of the QOL.

    Conclusion

    The quality of life of patients with CIED infection was lower than that of the normal population, and the factors influencing the quality of life were anxiety, depression, infection time and education.

    Protective effects and mechanisms of recombinant human erythropoietin on myocardial ischemia/reperfusion injury in rabbits

    Objective

    To investigate the influence of Recombinant Human Erythropoietin (rhEPO) on cardiac myocyte apoptosis and the expressing level of Caspase-3 protease which is critical in apoptosis in Myocardial ischemia/reperfusion injury(MI/RI) rabbits in order to explore the protective effects of rhEPO on MI/RI rabbit and its mechanisms of action.

    Methods

    Sixteen New Zealand rabbits were randomly divided into two groups: control group( MI/RI group) and drug group (rhEPO group). Rabbits were subjected to descending coronary artery(LAD) occlusion to establish rabbit MI/RI models.When the left anterior descending coronary arteries were ligated, RhEPO (1 000 U/kg) were injected in rabbits of rhEPO group, the rabbits of control group were injected saline equivalently. Serum abstracted from the blood for testing the concentration of CK-MB at the following points: 5 minutes before occlusion, 60 minutes after occlusion, 60 minutes and 180 minutes after reperfusion; At 180 minutes after reperfusion, The apoptotic index of the corresponding ischemic myocardial cell and the level of Caspase-3 in ischemic region were tested; The specimens of light microscope and electron microscopy were made to observe the general myocardial tissue morphosis and ultramicro organizational structure.

    Results

    The Serum CK-MB concentration increased gradually with protraction of the ischemia and reperfusion time(P<0.01). Before ischemia the serum CK-MB concentration were no significant difference between rhEPO group and the MI/RI group(P>0.05). The increased level of CK-MB concentration in rhEPO group were markedly lower than those in the I/R group at 60 minutes after ischemia, 60 minutes after reperfusion and 180 minutes after reperfusion.The difference was statistically significant(P<0.01). Compared to the MI/RI group, both the ischemic myocardial apoptotic index and the expressing level of Caspase-3 in rhEPO group decreased significantly(P<0.01). The impaired changes of the general morphosis and the ultrastructure at MI/RI areas in the MI/RI group were severer than that in the rhEPO group.

    Conclusion

    RhEPO has visible protective effects on the MI/RI rabbits. The mechanism of cardioprotection may be associated with stabilizing the myocardium cell membrane structure to decrease the release of myocardial enzyme and inhibit cardiac myocyte apoptosis. RhEPO can inhibit the cardiac myocyte apoptosis induced by ischemia/reperfusion injury through downregulation of the Caspase-3 protein expression.
