Abstract:
Objective To investigate the effect of atrial fibrillation bipolar radio-frequency ablation combined with prosthetic valve replacement on the postoperative immediate recovery rate of sinus rhythm and prognosis of cardiac function in patients with severe heart valve disease.
Methods Sixty-eight patients with severe valvular heart disease in our hospital from August 2014 to September 2016 were divided into 2 groups according to different surgical protocols with each 34 cases.The control group received prosthetic valve replacement, and the experimental group underwent prosthetic valve replacement combined with atrial fibrillation bipolar radio-frequency ablation.The operation conditions(total blood volume, intraoperative cardiac arrest time, postoperative immediate recovery rate of sinus rhythm and postoperative drainage volume) were compared between the two groups.Following up for 1 year, the changes of cardiac function indexes[left ventricular diameter, left ventricular ejection fraction (LVEF) and pulmonary artery systolic pressure] before and after operation were compared between the two groups.
Results There was insignificant difference between the two groups in total blood transfusion volume, intraoperative cardiac arrest time and postoperative drainage volume(P>0.05). The recovery rate of sinus rhythm was 91.18%(31/34) in the experimental group, higher than 17.65% (6/34) in the control group, and the difference was statistically significant(P<0.05). There was insignificant difference in the left atrial diameter, LVEF and pulmonary artery systolic pressure between the two groups before operation(P>0.05). Compared with the control group, the LVEF in the experimental group was higher, the left atrial diameter and pulmonary artery systolic pressure were lower, and the difference was statistically significant(P<0.05).
Conclusion Atrial fibrillation bipolar radio-frequency ablation combined with prosthetic valve replacement in the treatment of severe heart valve disease can effectively improve the recovery rate of immediate sinus rhythm, and significantly improve the prognosis of heart function.It’s worthy of promotion and application in clinical treatment.
