Retrospective analysis of early postoperative arrhythmias in patients with complete atrioventricular septal defect

Abstract:

Objective

To investigate clinical characteristics and influencing factors of early postoperative arrhythmias in patients with complete atrioventricular canal (CAVC).

Methods

The clinical data of patients with CAVC who underwent surgery in the Department of Cardiothoracic Surgery, Children's Hospital of Nanjing Medical University from November 2016 to December 2022 were retrospectively analyzed. Logistic regression model was used to analyze the influencing factors of early postoperative arrhythmia.

Results

A total of 100 children were included, the median age of which was 10(6,45)months, and 46.0%(46/100) were males. All the patients completed the operation successfully, of which 89 were treated with modified single-slice method and 11 with double-slice method. Arrhythmias occurred in 36 cases within 24 hours after operation, and 14 cases still had new arrhythmias 1 week after operation. The factors affecting the occurrence of arrhythmias include the age of operation (≤1 year, OR=0.248, 95%CI 0.065-0.954, P=0.042), other complex congenital heart disease (OR=10.000, 95%CI 2.606-38.377, P=0.001), Down syndrome (OR=6.311, 95% CI 1.908-20.872, P=0.003) and preoperative severe pulmonary hypertension (>45 mmHg, OR=5.467,95%CI 1.154-25.901, P=0.032).

Conclusion

Arrhythmia is a common complication in postoperative children with CAVC. Early prevention and intervention treatment are helpful to the early recovery of patients after operation.
