    The levels of cardiac markers and echocardiographic features in patients with chronic heart failure

    Objective

    To analyze the levels of cardiac markers and echocardiographic features in patients with chronic heart failure (CHF).

    Methods

    A total of 82 CHF patients admitted into the Second People’s Hospital of Taixing City from April 2017 to March 2018 were selected as CHF group, and 24 healthy volunteers were selected as control group. CHF patients were divided into four CHF subgroups according to cardiac function classification: 23 cases of CHF grade 1, 25 cases of CHF grade 2, 21 cases of CHF grade 3 and 13 cases of CHF grade 4. The levels of brain natriuretic peptide (BNP) and growth differentiation factor-15 (GDF-15) in peripheral blood were measured. The interventricular septal end-diastolic thickness (IVSTD), left ventricular posterior wall diastolic thickness (LVPWTD), left ventricular end-diastolic diameter (LVDD) and left atrial end-systolic diameter (LAD) were measured by echocardiography. Left ventricular ejection fraction (EF), cardiac output (CO) and cardiac index (CI) were calculated by Simpson formula. Mitral flow spectra were obtained from apical four-chamber section. The maximum early diastolic velocity (E peak), late diastolic maximum velocity (A peak), E/A, early diastolic rapid filling fraction (RFF), left ventricular mass index (LVMI), relative wall thickness (RWT) were collected and compared between the two groups.

    Results

    Compared with the control group, the levels of BNP and GDF-15 in peripheral blood of CHF group increased significantly (P<0.05). The levels of BNP and GDF-15 in peripheral blood increased with the decrease of cardiac function (P<0.05). Compared with the control group, IVSTD, LVPWTD, LVDD, LAD, LVMI, RWT, CO and A in CHF group increased significantly (P<0.05). CI, EF, E/A and RFF decreased significantly (P<0.05). IVSTD, LVPWTD, LVDD, LAD, LVMI, RWT, CO and A also increased significantly (P<0.05), CI, EF, E/A and RFF also decreased significantly (P<0.05).

    Conclusion

    The cardiac markers BNP and GDF-15 of CHF increased significantly, and the indexes of echocardiography were abnormal. The abnormal mainly manifestation was left ventricular failure.

    Differential analysis of cardiac ultrasonic features in patients with different types of cardiomyopathy

    Objective

    To observe the differences of cardiac ultrasonic features in patients with different types of cardiomyopathy.

    Methods

    From January 2017 to September 2018, 55 patients with dilated cardiomyopathy (DCM group) and 55 patients with ischemic cardiomyopathy (ICM group) were enrolled in the study. All patients underwent echocardiographic examination. The echocardiographic parameters such as LVEDD, LVESD, LAD, RAD, RVD, EF, FS, SV and the changes of cardiac morphology were compared between the two groups.

    Results

    The levels of LVEDD, LVESD, LAD, RAD, RVD and SV in ICM group were lower than those in DCM group, and the levels of EF and FS were higher than those in DCM group (P<0.05). The incidence rates of left ventricular " spherical" shape change and diffuse wall motion reduction in DCM group were higher than those in ICM group, and the incidence rates of left ventricular " arcuate" shape change, segmental wall motion abnormality, round and thin apex of left ventricle, calcification of aortic root were higher than those in ICM group. The incidence rate of root calcification in ICM group was lower than that in ICM group (P<0.05).

    Conclusion

    There were significant differences in echocardiographic parameters and cardiac morphology between DCM and ICM. Most patients with DCM had both left and right ventricles enlarged at the same time, while patients with ICM had more severe aortic valve calcification.

    Echocardiographic diagnosis of primary cardiac tumors in infants and young children

    Objective

    To explore the value of echocardiography in the diagnosis of primary cardiac tumors in infants and young children.

    Methods

    Twenty-six infants suspected as primary cardiac tumors were selected and examined by echocardiography. After completing the relevant examinations, the diagnosis was confirmed by operation or puncture biopsy. The diagnostic value of echocardiography in primary cardiac tumors was analyzed.

    Results

    In 26 children suspected as cardiac tumors, 1 was diagnosed as primary cardiac tumors, belonging to cardiac myxoma (benign); 25 were diagnosed as endocarditis, of which 19 were associated with valvular vegetation. Two cases of primary cardiac tumors were diagnosed by echocardiography. One case of endocarditis was misdiagnosed as cardiac tumors. The diagnostic sensitivity and specificity were 100.0% and 96.0% respectively. The coincidence rate with pathological diagnosis was 96.15%.

    Conclusion

    Echocardiography has a high sensitivity in the diagnosis of primary cardiac tumors in infants and young children, which can provide objective basis for correct clinical diagnosis and is conducive to the formulation and implementation of treatment programs.

    Echocardiography for evaluating the short-term efficacy of emergency and delayed PCI

    Objective

    To explore the value of echocardiography in the evaluation of short-term efficacy of emergency percutaneous coronary intervention (PCI) and delayed PCI.

    Methods

    Sixty-two patients with acute myocardial infarction were selected and treated with PCI. They were grouped according to the operation time, 31 cases in each group. The control group received delayed PCI, while the observation group received emergency PCI. The changes of left atrium, left ventricular diameter and left ventricular function at different time points after operation were observed and compared between the two groups.

    Results

    The cardiac function indexes of the two groups 1 month and 3 months after operation were higher than those 1 week after operation, and the left ventricular inner diameter was smaller than that 1 week after operation (P<0.05).

    Conclusion

    Cardiac function improved significantly 1 month and 3 months after emergency PCI and delayed PCI, and the short-term curative effect was good.

    Objective

    To observe the effect of intravascular ultrasound (IVUS) in the interventional treatment of chroinc total occlusion (CTO).

    Methods

    A total of 72 CTO patients admitted to The Second Affiliated Hospital of Zhengzhou University from August of 2015 to March of 2017 were randomly divided into control group and experimental group, 36 patients in each group. The control group received PCI under the guidance of coronary angiography, and the experimental group received ivus-guided PCI. The surgical success rate, the incidence of intraoperative complications and the incidence of postoperative MACE were compared between the two groups.

    Results

    The operation success rate of the control group (75.50%) was lower than that of the experimental group (94.44%), and the difference was statistically significant (P<0.05). The complication rate of the experimental group (8.33%) was lower than that of the control group (27.78%), and the difference was statistically significant (P<0.05). The incidence of postoperative MACE in the experimental group (5.56%) was lower than that in the control group (22.22%), and the difference was statistically significant (P<0.05).

    Conclusion

    The IVUS has a good application effect in the interventional treatment of CTO patients, which can effectively improve the success rate of surgery, reduce the incidence of intraoperative complications and postoperative MACE, and has clinical promotion value.

    Diagnostic Significance of Cardiac Color Doppler Ultrasound in Patients with Left Ventricular Hypertrophy and Left Heart Failure

    Objective

    To explore the heart of color Doppler ultrasound for diagnosis of left heart failure patients with hypertensive left ventricular hypertrophy.

    Methods

    From September 2015 to March 2017 in our hospital 45 cases of hypertensive left ventricular hypertrophy with left heart failure patients as study group, and select the same period 45 cases of healthy and free of cardiovascular disease patients in China Institute of health as a group, echocardiography of two groups of patients, compare the LVEF, LVEDd, LVESd, systolic blood pressure, diastolic blood pressure and LVM, LVFS and other indicators, and different cardiac function classification in patients with LVEF, LVEDd and L AD index.

    Results

    Health groups were LVEF, LVEDd, LVESd, systolic blood pressure, diastolic blood pressure and LVM, LVFS and other indicators were significantly better than the study group (P<0.05), and with the increase of the LVEDd classification of cardiac function, LAD index increased significantly (P<0.05), while the LVEF index is obvious decreased (P<0.05).

    Conclusion

    The heart color Doppler ultrasound detection based on left ventricular hypertrophy in hypertension patients with left heart failure, to observe the cardiac structure and ejection level, and according to the index of diagnosis, early detection of the disease, early treatment.It is significant and worthy of promotion.

    Analysis of the effect of Ultrasound Combined with Electrocardiogram in the Diagnosis of Hypertension Heart Disease

    Objective

    The application of ultrasound combined with electrocardiogram in the diagnosis of hypertensive heart disease (EH) was discussed.

    Methods

    From June 2015 to June 2017, 64 patients with hypertensive heart disease admitted in Jiaozhou people′s Hospital were randomly divided into control group (n=32) and observation group (n=32). The control group was diagnosed by electrocardiogram, the observation group was diagnosed by ultrasonic combined electrocardiogram, and the diagnosis was compared between the two groups.

    Results

    The detection rate of the observation group was 81.25%, which was significantly higher than that of the control group(53.13%).

    Conclusion

    The application of ultrasound combined with electrocardiogram in the diagnosis of hypertensive heart disease has a remarkable effect, which can improve the detection rate, provide accurate diagnostic data for the doctor, and be helpful for the patients to carry out the correct treatment plan, which is worthy of application.
