心力衰竭(heart failure)简称心衰,是指由于心脏的收缩功能和(或)舒张功能发生障碍,不能将静脉回心血量充分排出心脏,导致静脉系统血液淤积,动脉系统血液灌注不足,从而引起心脏循环障碍症候群,此种障碍症候群集中表现为肺淤血、腔静脉淤血。心力衰竭并不是一个独立的疾病,而是心脏疾病发展的终末阶段。其中绝大多数的心力衰竭都是以左心衰竭开始的,即首先表现为肺循环淤血。

    The levels of cardiac markers and echocardiographic features in patients with chronic heart failure

    Objective

    To analyze the levels of cardiac markers and echocardiographic features in patients with chronic heart failure (CHF).

    Methods

    A total of 82 CHF patients admitted into the Second People’s Hospital of Taixing City from April 2017 to March 2018 were selected as CHF group, and 24 healthy volunteers were selected as control group. CHF patients were divided into four CHF subgroups according to cardiac function classification: 23 cases of CHF grade 1, 25 cases of CHF grade 2, 21 cases of CHF grade 3 and 13 cases of CHF grade 4. The levels of brain natriuretic peptide (BNP) and growth differentiation factor-15 (GDF-15) in peripheral blood were measured. The interventricular septal end-diastolic thickness (IVSTD), left ventricular posterior wall diastolic thickness (LVPWTD), left ventricular end-diastolic diameter (LVDD) and left atrial end-systolic diameter (LAD) were measured by echocardiography. Left ventricular ejection fraction (EF), cardiac output (CO) and cardiac index (CI) were calculated by Simpson formula. Mitral flow spectra were obtained from apical four-chamber section. The maximum early diastolic velocity (E peak), late diastolic maximum velocity (A peak), E/A, early diastolic rapid filling fraction (RFF), left ventricular mass index (LVMI), relative wall thickness (RWT) were collected and compared between the two groups.

    Results

    Compared with the control group, the levels of BNP and GDF-15 in peripheral blood of CHF group increased significantly (P<0.05). The levels of BNP and GDF-15 in peripheral blood increased with the decrease of cardiac function (P<0.05). Compared with the control group, IVSTD, LVPWTD, LVDD, LAD, LVMI, RWT, CO and A in CHF group increased significantly (P<0.05). CI, EF, E/A and RFF decreased significantly (P<0.05). IVSTD, LVPWTD, LVDD, LAD, LVMI, RWT, CO and A also increased significantly (P<0.05), CI, EF, E/A and RFF also decreased significantly (P<0.05).

    Conclusion

    The cardiac markers BNP and GDF-15 of CHF increased significantly, and the indexes of echocardiography were abnormal. The abnormal mainly manifestation was left ventricular failure.

    Objective

    To investigate the influence of warfarin anticoagulant therapy on electrocardiogram and blood NT-proBNP level in patients with chronic heart failure.

    Methods

    A total of 120 patients with chronic heart failure admitted to Jiaozhou People's Hospital from September 2015 to December 2018 were selected. All patients were divided into control group and observation group with 60 cases each by random number table method. The control group received routine anti-heart failure treatment, while the observation group received warfarin treatment on the basis of the control group. The changes of left ventricular ejection fraction (LVEF), left ventricular end-diastolic diameter (LVEDD) and blood NT-proBNP were compared between the two groups.

    Results

    After treatment, there was no significant difference in LVEDD between the two groups (P>0.05). LVEF in the observation group was higher than that in the control group (P<0.05). The levels of NT-proBNP in the two groups decreased after treatment, and the observation group had a larger change range, with a significant difference (P<0.05).

    Conclusion

    Warfarin can reduce the level of NT-proBNP in patients with chronic heart failure and improve cardiac function. It has significant clinical application value.

    Efficacy of perindopril combined with metoprolol on elderly patients with heart failure complicated with ventricular arrhythmia

    Objective

    To explore the efficacy of perindopril combined with metoprolol on elderly patients with heart failure complicated with ventricular arrhythmia.

    Methods

    According to the order of admission, 88 elderly patients with heart failure and ventricular arrhythmia were divided into control group and observation group, 44 cases in each group. The control group was treated with perindopril on the basis of routine treatment, while the observation group was treated with perindopril and metoprolol. The changes of heart rate, left ventricular ejection fraction and the incidence of adverse reactions were compared between the two groups before and after treatment.

    Results

    After treatment, the heart rate of the two groups decreased, left ventricular ejection fraction increased, the difference was statistically significant (P<0.05); after treatment, the heart rate of the observation group was lower than that of the control group, and left ventricular ejection fraction was higher than that of the control group, the difference was statistically significant (P<0.05). The incidence of adverse reactions in the observation group (4.55%) was lower than that in the control group (15.91%), and the difference was statistically significant (P<0.05).

    Conclusion

    For elderly patients with heart failure and ventricular arrhythmia, perindopril combined with metoprolol can significantly improve cardiac function with fewer adverse reactions, which is worthy of clinical application.

    Objective

    To observe the effect of humanized nursing intervention on quality of life and negative emotion in elderly patients with coronary heart disease (CAD) combined with chronic heart failure (CHF).

    Methods

    Ninety-four patients with CAD combined with CHF were divided into group A and B by random number table, 47 cases in each group. Group A received routine nursing and group B received humanized nursing. The MLWHFQ, SAS and SDS scores before and after nursing, and the nursing satisfaction scores were compared between the two groups.

    Results

    After nursing intervention, the MLWHFQ score in group B was lower than that in group A (P<0.05). The SAS and SDS scores in group B were lower than those in group A (P<0.05), and the nursing satisfaction score in group A was lower than that in group B (P<0.05).

    Conclusion

    Humanized nursing has positive effect on improving the quality of life and negative emotions of elderly patients with CAD combined with CHF. It can be popularized in clinic.

    Objective

    To explore the application value of holistic nursing intervention in perioperative period of cesarean section in patients with pregnancy complicated with heart failure.

    Methods

    From January 2016 to October 2018, 86 cases of pregnancy complicated with heart failure were selected and divided into observation group and control group according to admission order, 43 cases in each group. The control group received routine nursing during the perioperative period of cesarean section, while the observation group received holistic nursing intervention on this basis. Maternal and infant outcomes and nursing satisfaction were compared between the two groups.

    Results

    The Apgar score of the observation group was higher than that of the control group (P<0.05). The incidence of complications was 4.65% (2/43) in the observation group and 11.63% (5/43) in the control group, and there was no significant difference (P>0.05). The nursing satisfaction of the control group was 74.42% (32/43) and that of the observation group was 93.03% (40/43), and the difference was statistically significant (P<0.05).

    Conclusion

    Holistic nursing intervention is helpful to improve maternal and infant outcomes of pregnant women with heart failure, and nursing satisfaction is high.

    Objective

    To observe the effect of comprehensive nursing intervention on negative emotion and quality of life in elderly patients with chronic heart failure (CHF).

    Methods

    Ninety-two elderly patients with CHF were divided into control group and observation group by random number table, 46 cases in each group. The control group received routine nursing care, while the observation group received comprehensive nursing intervention. The scores of SAS, SDS and MLWHFQ before and after nursing were compared between the two groups, and the scores of nursing satisfaction were compared between the two groups.

    Results

    After nursing, the scores of SAS, SDS and MLWHFQ in the observation group were lower than those in the control group (P<0.05). The nursing satisfaction score of the control group was (78.93±5.54) scores and that of the observation group was (91.04±4.96) scores. The nursing satisfaction score of the observation group was higher than that of the control group (P<0.05).

    Conclusion

    Comprehensive nursing intervention can effectively improve the negative emotions and quality of life of elderly patients with CHF, and has clinical popularization value.

    Delivery mode and pregnancy outcome in pregnant women with heart failure

    Objective

    To investigate the delivery mode and pregnancy outcome in pregnant women with heart failure.

    Methods

    A total of 100 pregnant women with heart failure admitted to Jiaozhou People's Hospital from January 2017 to January 2018 were selected as the research objects, and the incidence of delivery modes (vaginal delivery and cesarean section) and adverse pregnancy outcomes (neonatal asphyxia, full-term low body mass, premature birth, perinatal death) were counted.

    Results

    Of the 100 pregnant women, 10 were NYHA grade Ⅰ, 49 were NYHA grade Ⅱ, 29 were NYHA grade Ⅲ and 12 were NYHA grade Ⅳ. The vaginal delivery rate of grade Ⅰ+ Ⅱ pregnant women was 52.54% (31/59), and that of grade Ⅲ+ Ⅳ pregnant women was 26.83% (11/41), and the difference was statistically significant (P<0.05). The incidence of neonatal asphyxia, full-term low body mass, premature delivery and perinatal mortality of grade Ⅰ+ Ⅱ pregnant women were lower than those of the control group, and the difference was statistically significant (P<0.05).

    Conclusion

    Heart function status of pregnant women with heart failure can have a significant impact on delivery mode and pregnancy outcomes. The higher the cardiac function classification, the higher the risk of cesarean section rate and adverse pregnancy outcomes.

    Effects of metoprolol combined with trimetazidine on the serum levels of inflammatory factor and immune function in elderly patients with coronary artery heart disease combined with heart failure

    Objective

    To observe the effect of metoprolol combined with trimetazidine on the serum levels of inflammatory factor and immune function in elderly patients with coronary artery heart disease combined with heart failure.

    Methods

    Ninety-four elderly patients with coronary heart disease and heart failure were randomly divided into group A and group B, 47 cases in each group. Group A received metoprolol on the basis of routine treatment, while group B received trimetazidine on the basis of group A. The total effective rate of the two groups was counted, and the levels of CRP, CD4+ , CD8+ before and after treatment were compared between the two groups.

    Results

    The total effective rate of group B (95.74%) was higher than that of group A (80.85%), and the difference was statistically significant (P<0.05). After treatment, the CRP level of both groups decreased significantly (P<0.05), while that of group B was lower than that of group A (P<0.05). After treatment, the CD4+ level of group B was higher than that of group A (P<0.05).

    Conclusion

    Metoprolol combined with trimetazidine in the treatment of elderly patients with coronary heart disease complicated with heart failure has a good clinical effect, can effectively reduce inflammation, improve immune function, and has clinical popularization value.

    Objective

    To observe the clinical effect of yangxin shengmai granule on elderly patients with coronary heart disease (CHD) complicated with chronic heart failure (CHF).

    Methods

    Ninety-two elderly CHD patients with CHF admitted to the Second People's Hospital of Jiaxian from January 2017 to June 2018 were included in the study. They were divided into group A and group B by random number table method, 46 cases in each group. Patients in group A received routine treatment, while patients in group B received yangxin shengmai granule. The cardiac function indexes such as LVEDD, LVESD, LVESV and LVEF were compared between two groups before and after treatment. The total effective rate and the incidence of adverse reactions of the two groups were counted.

    Results

    After treatment, the levels of LVEDD, LVESD and LVESV in group B were lower than those in group A, and the levels of LVEF in group B were higher than those in group A (P<0.05). The total effective rate in group B (93.48%) was higher than that in group A (80.43%). There were significant differences between two groups (P<0.05). There was no significant adverse reaction during treatment in both groups.

    Conclusion

    Yangxin shengmai granule has a good clinical effect in the adjuvant treatment of CHD complicated with CHF in the elderly, which can effectively improve the cardiac function of patients without increasing the risk of medication, and has a high clinical popularization value.

    Objective

    To observe the clinical efficacy of trimetazidine combined with metoprolol in the treatment of senile coronary heart disease (CHD) combined with heart failure (CHF).

    Methods

    A total of 110 elderly patients with CHD combined with CHF admitted to Xinzheng People’s Hospital from May 2017 to May 2018 were randomly divided into control group and observation group, 55 patients in each group. Patients in group A received trimetazidine on the basis of routine treatment, and patients in group B received metoprolol on the basis of treatment in group A. The levels of inflammatory factors such as LVEF, LVEDD, LVESD and the inflammatory factors such as IL-6, TNF-α were compared before and after treatment in the two groups.

    Results

    The total effective rate of the observation group (98.11%) was higher than that of the control group (81.82%), and the difference was statistically significant (P<0.05). The level of LVEF after treatment in the observation group was higher than that in the control group, and the levels of LVEDD and LVESD in the observation group were lower than those in the control group (P<0.05). The levels of IL-6 and TNF-a after treatment in the observation group were lower than those in the control group (P<0.05).

    Conclusion

    Trimetazidine combined with metoprolol in the treatment of elderly CHD with CHF has a good clinical effect, can effectively improve the cardiac function of patients, and can inhibit inflammation. It has clinical popularization value.

    Objective

    To observe the therapeutic effect of Shenfu Yixin granule in the treatment of patients with chronic congestive heart failure (CHF) combined with pulmonary hypertension (PH).

    Methods

    A total of 62 patients with CHF combined with PH admitted to Henan People's Hospital from January 2016 to December 2017 were selected and randomly divided into control group and experiment group, 31 cases in each group. The control group was treated with conventional Western medicine, and the experimental group was treated with Shenfu Yixin granule for one month. Pulmonary artery systolic pressure (PASP), left ventricular ejection fraction (LVEF) and stroke volume (SV) were compared between the two groups before and after treatment.

    Results

    After treatment, the level of PASP in the two groups decreased compared with that before treatment, and the level of PASP in experimental group was lower than that in control group (P<0.05). After treatment, the levels of SV and LVEF in the two groups were higher than those before treatment, and the levels of SV and LVEF in experimental group was higher than that in control group (P<0.05).

    Conclusion

    Shenfu Yixin granule can reduce PASP and improve cardiac function in patients with CHF combined with PH.

    Objective

    To study the effect of levocarnitine combined with captopril in the treatment of dilated cardiomyopathy complicated with heart failure.

    Methods

    A total of 92 patients with dilated cardiomyopathy complicated with heart failure treated in Jiaozhou People's Hospital from April of 2016 to December of 2017 were selected and divided into control group and observation group according to the treatment plans, 46 cases in each group. Both groups received routine anti-heart failure treatment. The control group received levocarnitine injection treatment, and the observation group received levocarnitine injection combined with captopril treatment. Left ventricular ejection fraction (LVEF), left ventricular end-systolic diameter (LVESD), serum brain natriuretic peptide (BNP), transforming factor-β1 (TGF-β1) levels and MLHFQ scores were compared between the two groups before and after treatment.

    Results

    After treatment, the LVEF of the observation group was higher than that of the control group, and the LVESD of the observation group was lower than that of the control group (P<0.05). After treatment, the serum BNP level of the observation group was lower than that of the control group, and the serum TGF-β1 level was higher than that of the control group (P<0.05). After treatment, the MLHFQ score of the observation group was lower than that of the control group (P<0.05).

    Conclusion

    In the treatment of dilated cardiomyopathy complicated with heart failure, levocarnitine injection combined with captopril can effectively improve cardiac function, reduce serum BNP level, improve the level of TGF-β1, and improve the quality of life of patients with dilated cardiomyopathy

    Clinical efficacy of low-dose spironolactone in the treatment of senile coronary heart disease with chronic heart failure

    Objective

    To observe the clinical efficacy of low-dose spironolactone in the treatment of advanced coronary heart disease (CAD) with chronic heart failure (CHF).

    Methods

    A total of 84 patients with CAD and CHF admitted to Dancheng People's Hospital from September of 2016 to June of 2017 were included in the study. They were divided into two groups according to the random number table method, 42 cases in each group. Patients in group A received routine treatment, while patients in group B received low-dose spironolactone adjuvant therapy on the basis of routine treatment. The total effective rate, the level of LVEF, NT-proBNP before and after treatment and the incidence of adverse reactions were compared between the two groups.

    Results

    The total effective rate of group A was 78.57% and that of group B was 95.24%. The total effective rate of group B was higher than that of group A, and the difference was statistically significant (P<0.05). After treatment, the level of LVEF in group B was higher than that in group A (P<0.05), and the level of NT-proBNP was lower than that in group A (P<0.05). The incidence of adverse reactions was 9.52% in group A and 11.90% in group B. There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).

    Conclusion

    Low-dose spironolactone has a good clinical efficacy in the adjuvant treatment of CAD combined with CHF, which can effectively improve the cardiac function of patients, and does not increase the risk of drug use, so it can be popularized in clinic.

    Objective

    To investigate the effect of verapamil combined with deacetylflorin injection on coronary heart disease with rapid atrial fibrillation complicated by acute heart failure.

    Methods

    A total of 112 patients with coronary heart disease with rapid atrial fibrillation complicated by acute heart failure were selected from Jiaozhou People's Hospital from March of 2016 to December of 2017 were selected and randomly divided into experimental group (56 cases) and control group (56 cases). On the basis of routine treatment, the control group was treated with deacetylflorin injection, while the experimental group was treated with verapamil combined with deacetylflorin injection. After 3 days of treatment, the clinical effect, incidence of adverse reactions, heart rate, diastolic blood pressure and systolic blood pressure before and after treatment were compared between the two groups.

    Results

    After 3 days of treatment, the total effective rate (96.43%) of the experimental group was higher than that of the control group (78.57%) (P<0.05); the heart rate, diastolic blood pressure and systolic blood pressure of experimental group were lower than those of control group (P<0.05). The incidence of adverse reactions of the experimental group (10.71%) was not significantly different from that of the control group (5.36%) (P>0.05).

    Conclusion

    Verapamil combined with deacetylflorin injection is effective in the treatment of patients with coronary heart disease with rapid atrial fibrillation complicated by acute heart failure. It can improve the heart rate and blood pressure of patients with high safety.

    Objective

    To investigate clinical effect of metoprolol combined with trimetazidine in the treatment of coronary heart disease with heart failure.

    Methods

    Eighty patients with CHF treated in Jieshou People's Hospital from July of 2016 to June of 2017 were divided into control group and experimental group according to the treatment methods. The control group received routine treatment, while the experimental group received metoprolol and trimetazidine on this basis. The therapeutic effects of the two groups were compared.

    Results

    The total effective rate of experimental group was 92.5%, higher than that of control group (75.0%), and there was significant difference between the two groups (P<0.05). After treatment, LVESD, LVEF and LVEDD of experimental group were better than those of control group (P<0.05).

    Conclusion

    Metoprolol combined with trimetazidine has a significant effect in the treatment of coronary heart disease with heart failure, can effectively improve the cardiac function of patients, is worthy of clinical promotion.

    Objective

They were randomly divided into control group and observation group, 49 cases in each group. The control group received routine nursing, while the observation group received holistic nursing. The length of hospitalization, the score of nursing satisfaction and the effect of treatment were compared between the two groups.

    Results

    The length of hospitalization in the control group was (18.08±3.63) d, and the nursing satisfaction score was (85.17±3.48) scores. The length of hospitalization in the observation group was (9.52±2.41) d, and the nursing satisfaction score was (98.36±0.19) scores. The treatment time of the observation group was longer than that of the control group, and the nursing satisfaction score was higher than that of the control group, and the differences were statistically significant (P<0.05). The total effective rate in the control group was 79.59% (39/49), and the total effective rate in the observation group was 97.96% (48/49). The total effective rate of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05).

    Conclusion

    In the treatment of patients with pulmonary tuberculosis complicated with heart failure, holistic nursing can help to improve the clinical therapeutic effect and the satisfaction of nursing, and promote the recovery of patients'condition, which is worthy of popularization and application.

    Objective

    To investigate the characteristics and risk factors of heart failure in elderly critically ill patients.

    Methods

    The clinical data of 100 elderly critically ill patients treated in emergency department of the Affiliated Hospital of Xuzhou Medical University from January of 2016 to July of 2017 were retrospectively analyzed. All patients were divided into 65 patients without heart failure and 35 patients with heart failure, and the characteristics of heart failure and related risk factors were analyzed.

    Results

    The age, incidence rates of coronary heart disease, diabetes mellitus, respiratory failure, renal insufficiency, valvular heart disease in patients with heart failure were higher than those in patients without heart failure (P<0.05). Logistic regression analysis showed that coronary heart disease, diabetes mellitus, respiratory failure, renal insufficiency, valvular heart disease were risk factors for heart failure in critically ill elderly patients.

    Conclusion

    Coronary heart disease, diabetes mellitus, and respiratory failure are risk factors for heart failure in elderly critically ill patients. When treating patients with this disease, comprehensive consideration should be given to their underlying diseases.

    Objective

    To observe the effect of grading matching combined with dynamic stepwise incremental exercise rehabilitation nursing in patients with chronic heart failure.

    Methods

    One hundred and fifty patients with chronic heart failure hospitalized in Changqing Oilfield Staff Hospital from January to August in 2017 were divided into two groups according to random number table method, seventy-five patients in each group. The control group received routine exercise rehabilitation nursing, and the observation group received graded matching + dynamic stepwise incremental exercise rehabilitation nursing. Results of six-minute walking test, the quality of life scores the effective rate of cardiac rehabilitation and re-admission rate within six months were compared between two groups.

    Results

    The result of six-minute walking test after intervention in the observation group was higher than that in the control group, and the quality of life score was lower than that in the control group (P<0.05). The effective rate of cardiac function rehabilitation in the observation group was higher than that in the control group, and the rate of re-hospitalization within 6 months was lower than that in the control group (P<0.05).

    Conclusion

    Graded matching combined with dynamic stepwise incremental exercise rehabilitation nursing can obtain better effect of cardiac function rehabilitation on patients with chronic heart failure, reduce re-admission rate and improve quality of life.

    Objective

    To analyze the effect of enalapril maleate folic acid tablet in the treatment of elderly patients with chronic heart failure on the level of NT-pro BNP and Hcy.

    Methods

    A total of 60 elderly patients with chronic heart failure admitted to our hospital from May 2016 to May 2017 were selected.The patients were divided into 2 groups according to the random number table method.The study group was treated with enalapril maleate folic acid tablets.The control group was treated with enalapril.The levels of NT-pro BNP and Hcy in patients after treatment were analyzed, and the levels of NT-pro BNP and Hcy before and after treatment were observed in patients with different grades of cardiac function.

    Results

    After treatment, NT-pro BNP and Hcy levels in the study group were lower than those in the control group (P<0.05), and the difference was significant.Before treatment, NT-pro BNP and Hcy levels increased with the cardiac functional class.Increase(P<0.05), significant difference.

    Conclusion

    The treatment of elderly patients with chronic heart failure with enalapril maleate folic acid tablets can improve the patients′ NT-pro BNP and Hcy levels.

    Objective

    To analyze the level of plasma brain natriuretic peptide (BNP) and the related factors in the patients with chronic heart failure after anti heart failure treatment.

    Methods

    The subjects selected from our hospital in 45 cases of elderly patients with chronic heart failure from October 2014 to December 2016 in Guizhou Cancer Hospital, the clinical data of all patients were analyzed retrospectively, and the patients as the observation group, and 45 healthy persons in our hospital were selected as control group in this study. Two groups of subjects of systolic blood pressure, diastolic blood pressure, pulmonary congestion, pulmonary hypertension, compare the left ventricular ejection fraction, and affecting factors of the related results were analyzed.Comparison of the patients in the observation group before after anti heart failure treatment and treatment level of BNP patients, and to assess the resistance the treatment of heart failure.

    Results

    There were significant differences in systolic blood pressure, diastolic blood pressure, pulmonary congestion, pulmonary hypertension and left ventricular ejection fraction between the two groups(P<0.05). The levels of BNP in the observation group were compared before and after treatment, and the difference was statistically significant before and after treatment (P<0.05).

    Conclusion

    Hypotension, pulmonary congestion and other related factors are independent risk factors of chronic heart failure. After the treatment of anti heart failure, the BNP level of patients can be significantly improved.
